Each school has a registered nurse (RN) and a School Health Room Technician (SHRT). The nurse is assigned to more than one school, so your child will most likely be seen by the SHRT who is at our school daily.
Listed below are a few guidelines to help ensure a healthy and positive learning experience for your child while they are in school:
What health information do I need to provide to my child’s school?
Health Inventory Form: All students entering a Maryland Public School are required to have a physical examination on file within 9 months of registration. Your health care provider may have a form or the Student Record Card 6 can be downloaded here or on the MCPS website. All incoming 6th students will need an updated Health Inventory Form.
Proof of Immunization: Because your child is required to have certain immunizations, they will not be able to attend school until a proof of immunization record is made available except, for religious or medical exemptions. The form is completed by the healthcare provider. If your child has been vaccinated in Maryland, you can visit the My IR website and download a copy for your records.
Student Emergency Information Form: This form is distributed in the beginning of every school year. It is available in ParentVUE and here. Please complete this form, sign and return as soon as possible. This information is very important to the health room staff in the event your child becomes ill or injured while at school. Notify the school when changes to phone numbers, emergency contacts or health concerns occur.
Dental Health Form: All students are required to provide a dental health form completed by your dentist. You can find it here (MCPS 525-17) or on the MCPS website. All incoming 6th students will need an updated Dental Health Form.

What if my child needs to take medication at school?
All medications whether prescribed or over the counter, must have a Medication Authorization form completed by the child’s healthcare provider, even if the child will self-carry the medication. This form is also available in the health room.
If your child requires medication during school hours, the parent/guardian and the healthcare provider must complete a Medication Authorization form (MCPS 525-13) or for Administration of Anaphylaxis Emergency Medication (MCPS 525-14). New forms are required at the start of every school year. Medication cannot be sent in your child’s backpack or with your child to school. Parents/guardians must deliver any prescribed or over the counter medication to the health room, in the original new container with the pharmacy label and be consistent with the physician order. All over the counter medication must be brought to the health room by the parents/guardians in a properly labeled, unexpired and unopened package. Students are not permitted to carry medications to and/from school. Parents will be notified at the end of the school year regarding picking up leftover medication from the Health Room.
What if my child needs a treatment during school?
If your child requires intermittent catheterizations or g-tube feedings during school hours, the parent/guardian and healthcare provider must complete an Authorization to Provide Medically Prescribed Treatment form (MCPS 525-12). If your child requires nebulizer treatments, the parent/guardian and health care provider will need to complete a Medication Authorization form (MCPS 525-13). Please note that during COVID, we are highly suggesting that your healthcare provider write for an inhaler and spacer, if needed, instead of prescribing nebulizer treatments.
What if my child has Asthma and needs to use an inhaler at school?
If your child requires the use of an inhaler during school hours, the parent/guardian and the healthcare provider must complete a Medication Authorization form (MCPS 525-13). New forms are required at the start of every school year. Medication cannot be sent in your child’s backpack or with your child to school. Parents/guardians must deliver any prescribed medication to the health room, in the original new container with the pharmacy label and be consistent with the physician order. What if my child has Diabetes?
If your child has diabetes, please set up a time to speak with the Nurse by emailing her at: jocelyn_gomezlopez@mcpsmd.org. Your child will need supplies to be delivered by a parent/guardian to the Health Room along with the Maryland Diabetes Medical Management Plan completed by their healthcare provider. The parent or guardian should complete the Diabetes Questionnaire and bring to the Health Room as well.
What if my child is sick or injured at school?
The health staff will evaluate all injuries and symptoms of illness. First aid will be provided as necessary and parents/guardians will be notified as appropriate. In case we are not able to locate either parent, the health room staff will contact the “relative or other individual to be notified in case of an emergency” as listed on the Student Emergency Information Form.
To help ensure a healthy experience at school and help prevent illness:
Please make sure your child gets enough rest. Studies have found that children require 8-12 hours of sleep per night. If your child does not eat breakfast at school, please make sure they eat a healthy breakfast before they leave home. Food is very important for energy and brain activity. Make sure your child washes their hands at appropriate times (i.e. before meals, after bathroom use, after playing, after sneezing/coughing into their hands, when visibly dirty, etc.) to cut down on the spreading of germs. Parents are encouraged to notify the health room when their child has lice, strep throat, chicken pox, pink eye, etc. so that the school can take precautionary measures. Any health information reported to the school is CONFIDENTIAL.
When should I keep my child home?
When your child is sick, they should stay home and not attend school. This not only benefits your child but other children and staff in the classroom at school.
