Transition Services: Helping students with disabilities make a successful transition to life after high school.
Transition services are defined in the Individuals with Disabilities Education Act (IDEA) as a coordinated set of activities for students that is designed within an outcome-oriented process, which promotes movement from school to post-school activities. These outcomes include:
- Postsecondary education
- Vocational training
- Integrated employment
- Continuing and Adult education
- Adult services
- Independent living
- Community participation
