In compliance with the Maryland Department of Health, parents of rising 6th graders must complete the attached health and immunization forms prior to the start of the 2022- 2023 school year. Please work with your healthcare provider to complete the attached forms. Forms may be completed and submitted electronically by email to our Redland Middle School Registrar, Mr. Johan Garcia at Johan_G_Garcia@mcpsmd.org.
Forms can also be hand delivered to the Redland Middle School main office.
