Affirmative Parental Consent Form
In accordance with the Family Educational Rights and Privacy Act, the Children’s Online Privacy Protection Act, and our MCPS regulations, Frost is seeking your consent to use approved online digital tools with your student. This process requires an opt-in, and parent consent for each student.
Please help us facilitate this process by completing the “Affirmative Parental Consent” Google Form in the email format of this message. One form must be completed for each Frost student in your family. Your consent allows our teachers to incorporate online digital tools in their lessons with your student(s).
