HEALTH AND EMERGENCY INFORMATION CARDS
At the beginning of the school year we send health forms and emergency information cards for you to complete. We must have up-to-date information on file in the office so that we can act in case of illness or emergency. Please be sure to notify the school of any changes during the school year.
MEDICATION POLICY
If your child requires medication (prescription or over-the-counter) during the day, MCPS policy requires the following:
• An Authorization to Administer Medication Form must be completed and signed by a doctor and the parent. This form is available at the office or online at www.montgomeryschoolsmd.org/departments/forms/student.shtm. If you do not have this form in the event of illness, a signed note of permission and instructions on your doctor's stationery are also acceptable.
• A parent or adult must bring the medicine to the office. Prescription medication must be received with proper label and instructions from the pharmacy, and the child must receive the first dose of prescription medicine at home. Over-the-counter medicine must be received in its original container with the manufacturer's label and safety seal intact.
• We will keep the medication in the Health Room and make sure your child receives the proper dose at the right time.
• Please note that the school cannot guarantee availability of persons who are trained to administer certain medications, such as use of an EPI pen in case of anaphylactic reactions.
These rules have been adopted to ensure the safe administration of medication for your child, and to make sure that medication does not accidentally get in the hands of any other children. We appreciate your cooperation. If you have any questions, please call our school nurse or Health Technician.
ILLNESS
If your child should be injured or become ill during the school day, the office will attempt to notify you at once. The child will rest in the Health Room until taken home. If neither parent can be reached, the school will call the person specified on the emergency information card. The school is not authorized to give any medicine, even Tylenol, unless we have written permission from the parent and a doctor.
ALLERGIES
Notify the school of all of your child's allergies.
GUIDELINES FOR KEEPING A CHILD HOME DUE TO ILLNESS:
• Children with any of the following symptoms should remain at home:
• Fever; listlessness, overall discomfort
• Diarrhea
• Vomiting
• Sore throat / swollen glands
• Undiagnosed rash (especially if pimply, scaly or if it involves most of the body)
• Visible signs of a cold (runny nose, cough)
BE SURE YOUR CHILD IS FREE OF FEVER FOR 24 HOURS BEFORE RETURNING TO SCHOOL.
LICE POLICY
Westbrook, like most schools, has had a recurring problem of students with head lice. WHEN LICE OR NITS ARE IDENTIFIED, TREATMENT WITH A SPECIAL LICE SHAMPOO IS ESSENTIAL. REGULAR SHAMPOOING WILL NOT ELIMINATE HEAD LICE.
Children with head lice will be sent home for treatment. When your child returns to school, you must bring him/her to the health room for inspection. All lice and/or nits within 1/2 inch of the scalp must be removed before your child may return to the classroom.
